Early stage pancreatic ductal neoplasia modelling with human embryonic stem cells

AbstractPancreatic ductal adenocarcinoma (PDAC) is one of the most lethal cancers with a dismal 5-year survival rate of less than 8%. Currently, there are no efficacious therapeutic approaches for advanced PDAC, calling for practical methods for early detection and intervention of PDAC. To date, it is largely unknown how this disease is initiated and develops in the pancreas. Despite various genetically engineered mouse models (GEMMs), there is a lack of models with normal human pancreatic exocrine cells to study the early stage of this disease. Having an in vitro model that faithfully recapitulates the disease development is thus important as data obtained from cancer cell lines cannot represent the malignant transformation process in the human pancreas. Human embryonic stem cells (hESCs) can serve as a vital source for generating pancreatic ductal-like organoids (PDLOs) through stepwise differentiation. KRASG12D is the most commonly mutated gene found in more than 90% of PDAC and take place early in most of the PDAC precursors. However, oncogenic KRAS activation alone only gives rise to the formation of pancreatic precursor lesions and infrequently drives the onset of PDAC, suggesting the participation of other players during the initiation and progression of this disease. In this study, we interrogate the function of our gene of interest during the initiation of PDAC by applying two inducible gene expression systems which allow tightly controlled temporal expression of our GOI on top of KRASG12D in PDLOs. Our study provides a practicable model to study the biological basis of PDAC initiation.
