Prognostic significance of combining VEGFa, FLT1 and KDR mRNA expressions in lung cancer

AbstractAngiogenesis plays an important role in cancer progression. Promising results in clinical trials have further proven the role of targeting VEGF signaling to prolong lung cancer patient survival. VEGFa has been demonstrated to be a prognostic marker in lung cancer in various studies, although prognostication using VEGF receptors, such as VEGFR1 (FLT1) and VEGFR2 (KDR) expression has produced varied results in different lung cancer studies. In the present study, we aimed to investigate the prognostic significance of these three factors, alone or in combination. Gene mRNA expression was extracted from four independent lung cancer cohorts totaling 583 patients and the association between gene expression and survival was investigated by statistical analyses. We found that when VEGFa, FLT1 and KDR expressions were considered alone, only VEGFa demonstrated a significant association with patient survival consistently across all 4 datasets. Patients with high expression of both VEGFa and either receptor had significantly worse survival than patients expressing low levels of VEGFa and the particular receptor. Importantly, we found that patients with a high level expression of all three genes in their tumor specimens had a significantly shorter survival compared to those patients with a low level expression of one, two or all three genes. Our results suggest that a high level expression of VEGFa and its receptors may be required for cancer progression and that these three factors should be considered together as a prognostic indicator for lung cancer patients.
